Release and Consent Agreements

Media Release Consent
By registering my child, I give Cache Children's Choir and its licensees the absolute right and permission to use, publish, re-use, re-publish, and distribute any photograph/voice/indicia or video footage in which my child may appear, in any fashion for any purposes in all media now known or hereafter devised without any limitations whatsoever, including but not limited to internet, web sites, Youtube, social media, etc., as well as unlimited use in any advertising medium, brochures, promotions and educational training whatsoever, without restriction. I further release Cache Children's Choir from any and all claims arising out of or in connection with the use of any photos/voice/indicia and video footage, including libel, slander, invasion of right of privacy, publicity or personality relating to the exercise of any rights referred to herein.

Child’s Health
I hereby certify that my child is in normal health and, to my knowledge, is capable of participating safely in this activity. Should any injury occur to my child during participation in Cache Children's Choir, I authorize Cache Children's Choir or Utah State University personnel to arrange for or provide emergency medical treatment and to arrange for or provide transportation to the nearest medical facility. I also understand that Cache Children's Choir and Utah State University do not carry health insurance for participants in this activity.

Waiver and Consent
On behalf of my child, I assume all risks of my child's participation in this activity. I hereby release and agree to hold harmless all Cache Children's Choir and Utah State University officers, directors, employees, and all volunteers from any liability, loss, or damage actions, claims and demands which I now have or which may hereafter arise from my child's participation. I understand Cache Children's Choir and Utah State University, directors, employees and all volunteers assisting with the activity are not responsible for any personal items brought by my child.
